The name you submit should match exactly with your identification document, as it will be used for learner registration and the same will appear on your certificate. Please note that if any changes are required after the certificate has been issued, the learner will be responsible for paying the reprint cost.

All enrolment documentation will be processed based on the information provided in your submission form.

You are required to submit this enrolment form within 14 days. Timely submission is important to ensure your enrolment documentation is processed without delays. Please note that if the form is not received within 35 days of your enrolment date, you will be withdrawn from the course.
